Sasailalim sa beripikasyon ng LSWDO/MSWDO ang pamamahagi ng UCT Cash Card para sa mga benepisyaryo ng Listahanan Identified Poor (LIP) sa Montalban, Rizal.
Lahat ng pangalan na nasa LIPs ay ni-recognize ng Department of Social Welfare and Development (DSWD).
Para sa mga benepisyaryo magpunta sa MSWDO at magdala ng VALID ID.
Kung representative ang kukuha ng cash card o kung ang benepisyaryo ay pumanaw na (deceased), magdala ng ss:
• Proof of death (death certificate of deceased beneficiary)
• Proof of relationship (marriage contract, birth certificate)
• Proof of authority (SPA for deceased beneficiary)
• Valid ID of beneficiary and representative
Sa iba pang katanungan, makipag-ugnayan sa Municipal Social Welfare and Development Montalban.
