MGA DAPAT DALHIN:

A. REGULAR BENEFICIARIES

For the beneficiaries:

- 1 Valid ID(Government-issued IDs or Company ID) and 1 photocopy of Valid ID.

B. BENEFICIARIES WITH SPECIAL CASES

For representatives:

- Proof of relationship to the beneficiary (Marriage Contract, Birth Certificate, and Certificate of Cohabitation).

- 1 Valid ID(Government-issued IDs or Company ID) and 1 photocopy of the Valid ID of the beneficiary.

- 1 Valid ID(Government-issued IDs or Company ID) and 1 photocopy of the Valid ID of the representative.

- Authorization Letter issued by the UCT-RPMO staff.

For detained beneficiaries:

- Certificate of detention.

- Proof of relationship to the beneficiary (Marriage Contract, Birth Certificate, and Certificate of Cohabitation).

- 1 Valid ID(Government-issued IDs or Company ID) and 1 photocopy of the Valid ID of the beneficiary.

- 1 Valid ID(Government-issued IDs or Company ID) and 1 photocopy of the Valid ID of the representative.

- Authorization Letter issued by the UCT-RPMO staff.

For deceased beneficiaries.

- Warrant of Release from Liability issued by the C/MSWDO.

- Photocopy of Death Certificate.

- Proof of relationship to the beneficiary (Marriage Contract, Birth Certificate, and Certificate of Cohabitation).

- 1 Valid ID(Government-issued
